To join us for any session, program, retreat or ceremony, you will need to submit the 4 required items listed below in STEP #1, plus. . .

If you are planning to participate in a Sacred Plant Ceremony and/or in an 8-day or longer shamanic retreat, kindly COMPLETE THE APPLICATION FORM FOR YOUR PREFERRED PROGRAM OR RETREAT AT THE BOTTOM OF THE PAGE IN STEP #2.

If you are without software for completing the subject .pdf forms online, or if you do not have the means for downloading, scanning and sending them to us via email, you may want to click through to the website CONVERT PDF TO WORD. There, after you save each .pdf form to your device, you will be able to convert the .pdf into a Word document without charge and complete the form in Word. This allows you to then email each completed form to us as a Word attachment. Our email address is at the top of each form.

For anyone in the USA or Canada without a health insurance plan in place, International Volunteer Card at https://www.volunteercard.com/compareplans/ offers a suitable, effective, 60-day plan to anyone taking a trip of 100 miles or more for the purpose of non-profit organization projects. [That's us! Muaisa Hale Pule is an approved 501(c)3.] The plan, suitable for participant involvement, costs only $30 US and covers everything from accidental medical, dental and illness to trip delay and baggage loss or delay. Moreover, IVC has longer term plans as well, if you need one, and they donate a significant portion of their proceeds to humanitarian projects.

• • • APPLICATION FOR 1-DAY, 2-DAY or 3_DAY "HAWAIAKA" SACRED PLANT CEREMONY Complete and submit this form along with the WAIVER OF LIABILITY CLAIMS FORM + THE 4 ITEMS IN SECTION #1 ABOVE Based on a pre-enrollment of 2 - 6 participants (parties of 8 - 11 also accepted), ceremonies take place on New Year's Eve and on every consecutive Sunday evening throughout the year. Please submit this application form along with your signed WAIVER OF LIABILITY CLAIMS FORM + THE OTHER 3 ITEMS LISTED ABOVE IN SECTION #1.

Once you complete this individually-tailored work and "get" that you are Divine Eternal Energy having a temporal 3D experience, you will never need to take another seminar or workshop. You can, of course, go wherever you choose for the entertainment value-- but you will know once and for all how to access your own Inner Wisdom without needing to seek or require external validation.
